Adult Day Programs may offer recreational activities, outings,
therapies, transportation, support groups, etc. to maximize basic functional
skills and daily living
skills. One may choose to participate 1 to 6 days per week,
and the individual returns to his/her residence in the
evening. This is NOT a listing of day
treatment programs (outpatient, intensive therapy).

Integration of long-term, primary, acute and preventative care through the use of interdisciplinary teams who provide comprehensive, continuous care for the elderly, disabled, at-risk, and low-income populations.
Two programs in Milwaukee County:
PACE - Program for All-Inclusive Care for the Elderly
Partnership
Must be at least 55 and resident of Milwaukee County.
Based for one who is living in community and has long term nursing need
(ICF I & II, SNF). Must have disability determination or be a frail elder.
Must be Medicare or Medicaid eligible.
All-Inclusive medical care, including primary, dental and podiatry services at day centers. Home health care, homemaking services, psychiatric services, transportation, medications, adult day services. Also provide Family Care and Partnership services in Racine and Kenosha counties. Will soon be expanding to other counties such as Ozaukee, Washington, Sheboygan and Waukesha.
