Community Resources - Day Programs

Adult Day Programs may offer recreational activities, outings, therapies, transportation, support groups, etc. to maximize basic functional skills and daily living skills.  One may choose to participate 1 to 6 days per week, and the individual returns to his/her residence in the evening.  This is NOT a listing of day treatment programs (outpatient, intensive therapy), those programs can be found under the Post-acute, Inpatient and Outpatient Rehabilitation section.

Integration of long-term, primary, acute and preventative care through the use of interdisciplinary teams who provide comprehensive, continuous care for the elderly, disabled, at-risk, and low-income populations.

Two programs in Milwaukee County:
PACE - Program for All-Inclusive Care for the Elderly
Partnership- 18 years and above

Must be at least 55 and resident of Milwaukee County. Based for one who is living in community and has long term nursing need (ICF I & II, SNF). Must have disability determination or be a frail elder. 
Must be Medicare or Medicaid eligible. 
All-Inclusive medical care, including primary, dental and podiatry services at day centers. Home health care, homemaking services, psychiatric services, transportation, medications, adult day services. Also provide Family Care and Partnership services in Racine, Kenosha, Outagamie, Ozaukee, Washington and Waukesha counties. Family Care provided services in Calumet, Sheboygan and Walworth counties.
